The Youth Transition Services provides supportive and robust transition support for Young people, aged 15-25yrs who are leaving our care or a youth justice residence programme. The Youth Transition service understand that young people moving from Care spaces to Independence face some big challenges. The Transition Support Service has been designed together with young people, using insights gathered through a co-design process, focusing on the need of the young people.

Social workers in schools - Swis
School social workers are an integral link between school, home, and community in helping students achieve academic success. They work directly with whanau, students and school administration, providing support and behaviour development programmes for young people and their whanau, to ensure the young person is well supported in both the home and school environments. Te Aka Ora provide comprehensive wrap-around services for Teen Parents in the Tairawhiti-Gisborne area.
Our Teen Parent services include Whanau Support within Te Whare Whai Hua Teen Parent Unit, Teen Parent Intensive Case Worker and Teen Parent Supported Living.
